7th and 8th grade students will be able to pick up schedules and pay fees on August 11th from 9-3 at the KMS office.
All 7th grade students are required to have the Tdap vaccine (or declination statement) in order to pick up their schedule on this date.
The Kenton Hardin Health Department will be at the Kenton Middle School offering the 7th grade vaccines from 8-3pm.
Vaccines that will be offered to Kenton Middle School 7th graders are:
Tdap, Chickenpox #2, Gardasil or the HPV vaccine, and the Meningitis vaccine.
Please bring with you an insurance card and shot record.
To use INSURANCE (either private or Medicaid) we need date of birth and social for the insured.
If another parent or grandparent brings your child please sent a note with consent for the Kenton Hardin Health Department to administer the chosen vaccines.
If you have no insurance, each vaccine is $10 with a one-time $20 administration fee. Payment is due that day,if using a check or money order please make it payable to the Kenton Hardin Health Department or cash is accepted. A receipt will be provided.
For those choosing for their child not to receive this vaccine, a declination statement can be obtained at the Kenton Middle School office that day.
